The first thing you need to do is to repair the damage. First, remove the rotted materials and replace it. The next thing you want to do is to install the drain base. The tile floors are not water proof so if exposed to water, they can absorb water.
You should also ensure the mortar is mixed properly. An improperly mixed mortar can crumble and fall easily. Then, the next thing is to build a sloped mortar base. The slope will move all the water to the drain preventing standing water.
